Licensed Practical Nurse (LPN

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term or short-term facility setting.

• Administering medications, wound care, and performing skilled nursing procedures.
• Assisting residents with activities of daily living (ADLs) while maintaining their comfort, dignity, and safety.
• Supervising and directing nursing assistants and medication aides during shifts.
• Monitoring residents' physical and emotional status, reporting changes to physicians and families.
• Developing, implementing, and reviewing individualized care plans in collaboration with the interdisciplinary team.
• Completing accurate clinical documentation and coordinating resident admissions, transfers, and discharges.
• Participating in quality assurance, staff education, and regulatory activities.
• Performing additional duties as assigned by leadership.

• Current active LPN/LVN license in the practice state.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience is a plus.
• Experience in supervisory or charge nurse roles preferred.
• Strong clinical observation, medication management, and documentation skills.
• Knowledge of federal and state long-term care regulations.
• Effective communication skills, both verbal and written.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records and proficiency in Microsoft Office.
