Baystate Health - - Responsibilities:
Provides safety observation of patients and reports changes to the RN;
Documents patient status every 15 minutes and communicates observations to the RN;
Assists with activities of daily living (bathing, feeding, toileting, ambulating, turning, repositioning);
Maintains a safe environment and conducts one-to-one observation, including de-escalation;
Transports patients in wheelchairs and stretchers as required and follows RN directions
