Baystate Health - - Responsibilities:
Provides safety observation of the patient and intervenes as necessary;
Documents patient status every 15 minutes and communicates observations to the RN;
Assists with activities of daily living including bathing, feeding, toileting, ambulating, turning, and repositioning;
Maintains a safe environment and remains with the patient during one-to-one observation; de-escalates as needed;
Participates in mandatory in-services, validations, and staff meetings to maintain competency
