Life Path is seeking a Home Care Case Manager to evaluate needs, plan services, and coordinate with agencies to support elders in Franklin County and the North Quabbin area.
The full-time role (35 hr/wk) is based in Greenfield with a pay rate of $21.60/hour, plus generous benefits including PTO, health/dental/vision, Flex spending, EAP, 401k, and more. A 6% bilingual differential applies where relevant.
