DescriptionThe Center for Human Development (CHD) is seeking an experienced and creative Case Manager for its Two Rivers Recovery Center in Greenfield. Two Rivers is a program for women in early recovery from substance use who are pregnant or postpartum mothers with their infants. The program supports up to 25 women and 5 infants in long‑term (6 to 12 months) residential treatment using a comprehensive clinical service that is trauma‑informed and harm‑reduction oriented.
YourRole as a Case Manager
The Case Manager is responsible for the development and implementation of person‑centered treatment plans, facilitation of evidence‑based therapeutic groups and individual therapy, assistance with discharge planning, and coordination of community support and collaboration. The Case Manager receives supervision from a professional, experienced Clinical Director.
Requirements- High school diploma or equivalent required
- Knowledge of substance use disorders and treatment
- Experience facilitating groups
- Must pass background screening process including CORI
- Valid driver’s license and clear driving record check
- Reliable transportation for occasional business use

This is a full‑time, on‑site role. Tuesday – Saturday.
