Authora Care Collective is currently seeking a Hospice RN Home Care Case Manager for our Greensboro service area. This position is Full-time, 8:00am-5:00pm. CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG). Assess and identify patient/caregiver problems, establish nursing goals and interventions. Work with physician to establish medical plan of care for the patient. Establish and maintain therapeutic relationships with patient and caregiver, including providing emotional support and teaching. Communicate effectively with team members, patients, caregivers, physicians, pharmacists, and other health care professionals.
Document the outcome of care and provide dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

- Graduate of an accredited School of Nursing
- Must be currently licensed as an RN in North Carolina and in good standing with NCBON
- Experience in an acute, hospice, home health setting, or long-term care setting is a plus
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required
- Valid state-issued driver’s license
- Must carry automobile liability insurance at limits required by the agency
