About the Position

Conduct initial and ongoing assessments, monitor symptoms, and adjust the plan of care as needed .

Case manage a dedicated caseload, acting as primary RN and main point of contact for patients and families.

Coordinate with providers, social work, chaplains, and other team members to ensure holistic hospice care.

Document all visits, interventions, and changes in condition using HCHB (Homecare Homebase) EMR​.

Educate patients and caregivers on medications, symptom management, and end-of-life care expectations

Requirements

Minimum 1 year of recent  case management  experience in home health or hospice (must be willing to take and manage a caseload).

Reliable personal vehicle; comfortable traveling throughout the day to patient homes (this is  not  inpatient).

HCHB (Homecare Homebase) EMR experience required​.

Prior travel RN experience required.
