The Patient Services Coordinator‑LPN is directly responsible for scheduling visits and communicating with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day‑to‑day office and staff management.

Responsibilities
  • Manage schedules for all patients; edit schedules for agents calling in sick, ensuring patients are reassigned promptly; update agent unavailability in the worker console.
  • Initiate infection control forms as needed and send the HRD the completed “Employee Infection Report” to upload in the worker console.
  • Serve as backup during lunch hour and other busy times, including receiving calls from field staff and assisting with weekly case conferences; refer clinical questions to the Branch Director as necessary.
  • Maintain the client hospitalization log, entering coordination notes and sending the electronic log to all office, field, and sales staff.
  • Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.
  • Schedule all add‑ons and applicable orders, such as discharge visits, OASIS collection or recert visits following case conferences; schedule TIF OASIS collection visits and delete remaining schedules.
  • Reschedule declined or missed visits (when appropriate), process reassigned and rescheduled visits, and ensure supervisory visits are scheduled.
  • Run all scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report.
  • Prepare weekly Agent Schedules, perform initial review for productivity and geographic issues, and forward schedule to the Branch Director for approval before distribution to staff.
  • Verify visit paper notes in the scheduling console as needed; assist with internal transfer of patients between branch offices.
  • If clinical, receive lab reports and assess for normality, fax a copy to the doctor, make a copy for the Case Manager, and route to Medical Records; initiate Employee/Psychological Infection Reports as necessary.
  • If clinical, may be required to perform patient visits and/or participate in on‑call rotation.
Required Experience / Skills
  • Licensed Professional Nurse or Licensed Vocational Nurse, licensed in the state in which you practice.
  • At least 1 year of home‑health experience.
  • Prior packet review / QI experience preferred.
  • Code certification is preferred.
  • Valid state driver’s license and automobile liability insurance.
  • Must be currently licensed in the state of employment (if applicable).
  • Excellent communication skills; ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.

$48,900 - $66,200 per year
