Job SummaryProvides direct primary medical care to all participants of the PACE program.
SpecificationsEducation and Special Training: Graduate of an accredited School of Medicine, Residency in Internal Medicine or Family Practice. Additional training/Fellowship in Geriatrics preferred.
Experience: At least 2 – 3 years’ experience, minimum one-year experience working with the frail/elderly population.
Licensure, Registry or Certification
Required:
Current license to practice medicine in North Carolina (NC), valid NC driver’s license and vehicle.
Knowledge, Skills, and Abilities: Proven clinical leadership;
Ability to excel with others in a team environment;
Act within the scope of his or her authority to practice;
Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by CMS before working independently.
With or without reasonable accommodations:
Ability to bend and or stoop frequently;
Have independent mobility in order to climb stairs and walk distances;
Lift and position patients of all weights and heights utilizing correct body mechanics;
Lift and carry pieces of durable medical equipment;
Utilize correct body mechanics to transfer, lift and position participants;
Read, write, and speak in English;
Communicate effectively both orally and in writing;
Ability to clearly see medical records and various documents;
Hear blood pressures, and spoken communication;
Ability to perform repetitive hand/wrist movements.
- Performs comprehensive history and physical on new referrals.
- Performs interval history and physicals on established participants completed quarterly and as indicated.
- Evaluated and treats participants with episodic illnesses.
- Refers participants to medical specialists as indicated.
- Admits participants to the hospital:
- (a) Coordinates primary care with hospital based physicians;
- (b) Provides updates to the team on a regular basis;
- (c) Judiciously utilizes specialty consultants;
- (d) Responsible for coordinating discharge planning with the team.
- Manages all care of participants in the nursing home:
- (a) Provide regular visit;
- (b) Performs telephone contacts with nursing home staff as required;
- (c) Admits nursing home participants to hospital when necessary.
- Actively participates in family conferences.
- Coordination of 24 hour care delivery.
- Documents participant changes appropriately in the medical record and communicates participant change to team members.
- Participates as a member of the Interdisciplinary Team. Takes part in participant care planning.
- Perform all duties related to PCP position per regulatory requirements.
- Be accountable to PACE organization.
- Cooperate with the competency evaluation program and direct participant care requirements.
- Other duties as assigned.
Tasks with potential for exposure to blood borne pathogens include, but are not limited to the following:
- Conducting assessments of participants
- Performing procedures in the clinic
Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
