National Church Residences Home Health is seeking a dedicated Registered Nurse Case Manager to join our compassionate team in a full-time role. You will provide skilled nursing care and coordinate the overall care plan for hospice patients and their families.
This role covers South Franklin County and Madison County with on-site visits. An RN license and at least two years of RN experience are required; hospice or home health background is strongly preferred.
