Authora Care Collective is currently seeking a

Part-time;
Every other Saturday and Sunday 8:00am - 2:00pm.

Job Summary

Assess and identify patient/caregiver problems, establish nursing patient/caregiver care goals, and identify and implement interventions for all agency programs. Will document outcomes of care. Work with physician to establish and implement medical plan of care for the patient. Work closely with other members of the interdisciplinary group and communicate effectively both verbally and in writing with group members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations.

Establish and maintain therapeutic relationship with patient/caregiver to include providing emotional support and teaching.

Authora Care Collective is currently seeking a Hospice RN for our After-hours department, Home Care/Long Term Care (End of Life Visits) supporting our Greensboro service area
.

Part-time;

Requirements

Education and Experience
  • Graduate of an accredited School of Nursing
  • Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
  • One year of experience in an acute, hospice, home health setting, or long-term care setting plus.
Other:

Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
