Home Health LPN

Liberty Health is seeking a Home Health LPN to provide compassionate, evidence-based nursing care to adults in their homes. The LPN will perform assessments, administer medications and treatments, monitor chronic conditions, and report changes in status to the RN and physician. Responsibilities include wound care, patient education, documentation in electronic health records, and coordination with interdisciplinary behavioral health and social service teams.

This role supports person-centered care plans, ensures compliance with clinical and safety standards, and collaborates closely with families to promote optimal health outcomes in the community.

Responsibilities

• Provide in-home nursing care under RN and physician direction
• Perform patient assessments and monitor vital signs and chronic conditions
• Administer medications and treatments, including wound care
• Document care accurately in electronic health records
• Educate patients and families on disease management and medications
• Collaborate with interdisciplinary behavioral health and social service teams
• Report changes in patient condition promptly to supervising clinicians
• Support development and implementation of person-centered care plans
• Ensure compliance with infection control and safety standards
• Participate in continuous quality improvement activities
