Schedule: Monday–Friday, 8:00 AM–5:00 PM
Salary: $76,424–$102,706/year DOE + Benefits
We’re seeking an experienced RN Care Manager to provide nursing leadership and oversee care management services for a geriatric population. This role combines clinical leadership, staff management, care coordination, and quality improvement.
Key Responsibilities- Provide clinical and operational leadership for care management services
- Supervise Nurse Care Managers, Home Care Coordinators, Medical Assistants, and CNAs
- Support nursing assessments and care planning for geriatric patients
- Develop, implement, and evaluate policies, procedures, and projects
- Support staff education, training, and development
- Facilitate teamwork and interdisciplinary collaboration
- Manage complex situations and resolve clinical or operational concerns
- Prioritize multiple responsibilities and support quality improvement initiatives
- Graduate of an accredited nursing program
- Active North Carolina RN license
- 5+ years of managerial or supervisory experience
- Strong clinical leadership and nursing assessment skills
- Experience caring for geriatric patients
- Current BLS and First Aid certification
- Strong communication, organizational, and problem-solving skills
