Authora Care Collective seeks a Social Worker Home Care to support our Greensboro service area. This full-time role runs Monday–Friday, 8:00 a.m. to 5:00 p.m., with travel to patients’ homes as needed to ensure optimal care.
The Social Worker will perform generalist social work activities, including assessments, planning, intervention, case management, and resource coordination, with emphasis on long-term care planning and PASRRs.
