Amount of Travel RequiredWork may involve occasional travel between physician practice locations, hospital departments, community sites, and attendance at meetings or training sessions.

Work ScheduleFull-time days, Monday through Friday
Generally works scheduled physician practice operating hours. May be required to work occasional extended hours, evenings, or weekends based on clinic and organizational needs.
Work schedule may be adjusted as needed to meet operational and business requirements.
FLSA StatusNon-Exempt (Hourly); eligible for overtime pay in accordance with applicable law
Position SummaryThe Registered Nurse, Physician Practice provides professional nursing care within an outpatient physician practice setting. The RN serves as a clinical resource for providers, patients, and staff by performing patient assessment, triage, care coordination, population health activities, patient education, and clinical follow-up. The RN utilizes clinical judgment to support quality patient outcomes, care continuity, regulatory compliance, and value-based care initiatives while collaborating with providers, medical assistants, care coordinators, and other members of the healthcare team.
KNOWLEDGE,SKILLS, & ABILITIES/POSITION QUALIFICATIONS Education
Graduate of an accredited professional nursing education program required
Associate Degree in Nursing (ADN), Bachelor of Science in Nursing (BSN), or equivalent nursing education qualifying the individual for RN licensure required
ExperiencePrevious ambulatory care, physician practice, primary care, care coordination, or population health experience preferred
Computer SkillsProficient in Microsoft Office, email, and other standard computer applications, with the ability to learn and effectively use software systems required for assigned responsibilities.
Proficiency with electronic health records (EHRs)
Certificates & LicensesCurrent Registered Nurse license in the State of Indiana
Current BLS certification required
Other Requirements- Frequent use of computers, electronic medical records (EMR), telephones, and other office and clinical equipment.
- Ability to work effectively in a fast-paced environment, prioritize multiple competing responsibilities, manage frequent interruptions, and respond promptly to patient, provider, and operational needs.
- Ability to exercise sound clinical judgment, maintain attention to detail, and collaborate effectively as a member of an interdisciplinary healthcare team.
- Ability to adapt to changing priorities, manage fluctuating workloads and high patient volumes, respond appropriately to urgent clinical situations, and maintain productivity and quality of care.
- Use of appropriate personal protective equipment (PPE) and adherence to infection prevention and safety protocols.
- Conducts comprehensive assessments of patients' medical, behavioral, functional, and support needs to identify barriers to achieving optimal health outcomes.
- Develops, implements, and monitors individualized care plans in collaboration with patients, families, providers, and interdisciplinary team members.
