Licensed Practical Nurse

Interim Health Care is seeking a Licensed Practical Nurse to provide compassionate home-based care to patients across our community. In this role, you will deliver direct nursing care, including medication administration, wound care, vital signs monitoring, and chronic disease support, following individualized care plans. You'll collaborate closely with RNs, therapists, physicians, and families to promote safety, comfort, and independence  offer flexible schedules, strong clinical support, and ongoing training within a patient-first culture that values integrity, teamwork, and high-quality, personalized care.

Responsibilities

• Provide direct nursing care in patients' homes, including medications, treatments, and wound care
• Monitor and document vital signs, symptoms, and patient responses in EHR systems
• Follow individualized care plans under the direction of an RN or physician
• Educate patients and families on medications, disease management, and home safety
• Coordinate with interdisciplinary team members to support holistic patient care
• Ensure compliance with clinical protocols, infection control, and regulatory standards
• Report changes in patient condition promptly to supervising clinicians
• Travel to patient homes within the assigned service area
