Gentiva is seeking a Hospice RN / RN Case Manager to deliver patient-centered care in patients’ homes or care facilities. You will assess, plan, implement, and document skilled nursing care aligned with individualized plans of care.
You’ll collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members to ensure compassionate, comprehensive care and family education. A valid RN license and hospice experience are preferred.
