Ready to use your nursing experience in a leadership role? Greenville Nursing & Rehabilitation is seeking an experienced and detail-oriented RN MDS Coordinator to join our clinical leadership team.

This is a great opportunity for a nurse who understands MDS, enjoys working with the interdisciplinary team, and wants to play an important role in resident outcomes, care planning, reimbursement, and overall facility success.

Contract & Hours

Full-Time | Monday-Friday

Compensation & Incentives

Registered Nurse (RN): $39.00/hour
$10,000 Sign-On Bonus
Paid bi-weekly over one year
Plus an annual performance evaluation with the potential for a pay increase based on performance.

What You'll Do
  • Coordinate and oversee MDS assessments and submissions
  • Monitor assessment schedules and deadlines
  • Lead and participate in interdisciplinary care planning
  • Develop and update individualized resident care plans
  • Review clinical documentation for accuracy and completeness
  • Support reimbursement accuracy and quality measures
  • Maintain compliance with federal, state, and facility requirements
  • Support survey readiness
  • Serve as a resource for nursing staff regarding MDS and documentation
What We're Looking For
  • Current Illinois RN license
  • Previous MDS experience strongly preferred
  • Long-term care experience preferred
  • Knowledge of MDS processes, Medicare, Medicaid, and reimbursement
  • Strong organizational and time-management skills
  • Excellent communication and leadership abilities
  • Ability to work independently and collaboratively
  • EMR and clinical documentation experience preferred
