Ready to use your nursing experience in a leadership role? Greenville Nursing & Rehabilitation is seeking an experienced and detail-oriented RN MDS Coordinator to join our clinical leadership team.
This is a great opportunity for a nurse who understands MDS, enjoys working with the interdisciplinary team, and wants to play an important role in resident outcomes, care planning, reimbursement, and overall facility success.
Contract & HoursFull-Time | Monday-Friday
Compensation & IncentivesRegistered Nurse (RN): $39.00/hour
$10,000 Sign-On Bonus
Paid bi-weekly over one year
Plus an annual performance evaluation with the potential for a pay increase based on performance.

- Coordinate and oversee MDS assessments and submissions
- Monitor assessment schedules and deadlines
- Lead and participate in interdisciplinary care planning
- Develop and update individualized resident care plans
- Review clinical documentation for accuracy and completeness
- Support reimbursement accuracy and quality measures
- Maintain compliance with federal, state, and facility requirements
- Support survey readiness
- Serve as a resource for nursing staff regarding MDS and documentation
- Current Illinois RN license
- Previous MDS experience strongly preferred
- Long-term care experience preferred
- Knowledge of MDS processes, Medicare, Medicaid, and reimbursement
- Strong organizational and time-management skills
- Excellent communication and leadership abilities
- Ability to work independently and collaboratively
- EMR and clinical documentation experience preferred
