Position: Palliative Care Licensed Practical Nurse (LPN)
Ever Heart Hospice is seeking a compassionate and organized Palliative Care LPN to support patients and families facing serious or life-limiting illnesses. This position provides patient-centered care coordination, education, documentation, and admission support in collaboration with the palliative care and hospice teams.

Responsibilities Provide nursing care under the direction of an RN, Nurse Practitioner, physician, or provider in accordance with the patient’s care plan.

Monitor and report changes in patient condition, symptoms, or response to treatment.

Educate and support patients and caregivers regarding disease progression, treatment options, goals of care, and hospice or palliative care services.

Assist with scheduling follow-up calls, visits, labs, referrals, and care coordination needs.

Collaborate with physicians, Nurse Practitioners, admissions staff, referral sources, and interdisciplinary team members.

Complete accurate and timely documentation in the electronic medical record.

Assist with obtaining physician orders, admission paperwork, medication orders, DME coordination, and EMR data entry.

Follow up with patients in the Palliative Care and In Touch programs.

Support hospice and palliative care admissions, including pre-admission planning and communication with providers regarding eligibility and patient needs.

Serve as backup support to intake, admissions, and palliative care team members as needed.

Perform other duties as assigned.

Qualifications

Graduate of an approved School of Nursing.

Current LPN license in the State of Ohio.

Minimum of three years of hospice, medical/surgical, oncology, or community health nursing experience.

Palliative care experience preferred.

Valid driver’s license and ability to travel to patient homes, facilities, hospitals, and other sites as needed.

Ability to lift up to 25 lbs. and perform duties requiring sitting, standing, walking, bending, lifting, twisting, and turning.
