Role in Our Mission

Easterseals PORT Health’s Tailored Care Management (TCM) team is committed to delivering whole‑person, integrated care that truly makes a difference. As a Tailored Care Manager
, you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands‑on role with the opportunity to travel throughout Pitt, Greene and surrounding counties
, helping close gaps in care and improving lives in the community every day.

As a valued member of our team, you’ll earn a competitive salary ranging from $53,000 to $55,000 annually, based on your experience and qualifications. This full‑time role offers a primarily Monday–Friday schedule from 8 AM to 5 PM

What We’re Looking For

We’re seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after‑hours support.

Required Qualifications
  • Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area. Master’s level degree preferred.
  • Eligible for Qualified Mental Health Professional (QMHP) credentialing
  • Minimum 2 years of experience with mental health and/or substance use populations
  • Proficiency in Microsoft Office and Electronic Health Records (EHR)
  • Valid driver’s license, current auto insurance, and a clean driving record
  • Strong communication skills and the ability to work independently
