Pay range (depending on experience): $58–62/hr.
Schedule/Shift: Full Time, Monday‑Friday, Day Shift
Territory/

North OC territory
- Complete an initial assessment of the patient and family to determine home care needs, providing a complete physical assessment and history of current and previous illness(es).
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Regularly re‑evaluate patients’ needs.
- Use health assessment data to determine nursing diagnosis.
- Develop a care plan which establishes goals based on nursing diagnosis, incorporating therapeutic, preventive, and rehabilitative nursing actions, involving the patient and the family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
- Counsel the patient and family in meeting the plan of care and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
- Current and valid state RN license.
- Minimum of 1 year of recent acute care experience in an institutional setting.
- Basic Life Support (BLS) with hands‑on CPR certification.
- Valid driver’s license and auto insurance.
- Bachelor’s degree in Nursing, and 1 year of home health experience preferred.
