National Church Residences is seeking a Full Time Licensed Practical Nurse to provide direct nursing care and case management in home health across North Franklin County, Union County and Delaware County.
Schedule:
Monday–Friday, 8:00 am–4:00 pm with mileage reimbursement for travel between locations.
Responsibilities include implementing plans of care, documenting care, educating clients and families, coordinating services with the RN Case Manager, and mentoring direct care staff while on-site.
