Home Health Registered Nurse

Role Overview:

A Home Health Registered Nurse provides personalized, one-on-one care in patients' homes, collaborating with physicians and healthcare professionals to develop and implement individualized care plans, monitor patient conditions, educate families, and oversee home health aides.

• Administer medications, perform wound care, and conduct patient assessments onsite.
• Develop and update care plans in coordination with healthcare team.
• Monitor patient progress and report changes to physicians and managers.
• Educate patients and families on disease management and treatments.
• Maintain accurate care records and coordinate with healthcare providers.
• Supervise and ensure proper documentation by Home Health Aides.

• Experience in Med surg, ICU, ER, or acute care settings.
• Current, unrestricted RN license and CPR certification.
• Ability to collaborate effectively with healthcare teams.
• Valid driver’s license, auto insurance, and reliable transportation.
• Preferred: one year nursing and home health experience.

The role is patient-facing, requires TB screening, and adherence to driver safety policies. Compensation ranges from $77,200 to $106,200 annually, based on skills and location.
