Key Responsibilities

• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, and oral hygiene.
• Help residents with mobility, including turning, positioning, transferring, and ambulating using assistive devices.
• Feed residents or assist with meals, ensuring proper nutrition, hydration, and monitor intake and fluid consumption.
• Monitor and report changes in residents’ physical, emotional, or behavioral conditions to nursing staff.

• Measure and record vital signs as directed by nursing staff.
• Assist with monitoring skin conditions, reporting signs of skin breakdown or pressure sores.
• Assist with weight checks, intake and output monitoring, and other health-related measurements.

• Accurately document care provided and residents' responses in the facility’s electronic health record (EHR) or paper charts.
• Maintain records of food and fluid intake, toileting habits, and other observations.
• Report concerns or incidents to the supervising nurse and complete required documentation.

• Ensure resident safety by adhering to protocols and infection control guidelines.
• Assist in maintaining clean, organized, and comfortable living environment.
• Practice proper hand hygiene, use PPE as required, and follow all safety procedures.

• Provide compassionate care and emotional support, engaging in conversation and offering comfort as needed.
• Respect dignity, privacy, personal preferences, and promote independence where possible.
• Assist with social and recreational activities, encouraging participation to enhance quality of life.

• Work collaboratively with nursing staff, therapists, and other professionals to ensure comprehensive care.
• Participate in care plan meetings and contribute to individualized care plans.
• Assist with transportation of residents to appointments, therapy sessions, or activities within the facility.

Qualifications

• Education:
  
  High school diploma or equivalent required.
• Certification:
  Current and active State Tested Nursing Assistant (STNA) certification.
• Experience:
  
  Previous experience in healthcare, long-term care, or assisted living preferred but not required.
• Skills:
  
  Strong communication and interpersonal skills, compassionate, patient-centered approach.
• Ability to work effectively as part of a healthcare team.
• Basic understanding of medical terminology and ability to follow care instructions.
• Attention to detail and strong organizational skills.
• Proficiency in documenting care in electronic health record (EHR) systems is a plus.
