As an RN Case Manager
, you'll travel throughout Miami/Darke counties, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals –wherever they feel most comfortable.
- Active RN License
- Valid Driver's License
- 1+ years hospice experience
- BLS Certification
- Hospice and Palliative Medicine background
