Schedule RequirementsPRN/as needed
Must work at least 8 hours per month
Must work 1 weekend per quarter
Must work 1 holiday per year
Shifts:
6:00 AM–6:30 PM or 6:00 PM–6:30 AMHiring for both Day Shift and Night Shift
Key Responsibilities Provide skilled nursing care, symptom management, emotional support, and education to patients and families
Follow the established Plan of Care and physician orders
Perform initial and ongoing nursing assessments
Consult with physicians and/or the Medical Director as needed
Participate in interdisciplinary team planning and care coordination
Support admissions, referrals, chart reviews, and patient transfers as needed
Teach and supervise hospice aides, volunteers, and caregivers
Order appropriate Durable Medical Equipment (DME)
Complete timely and accurate documentation in compliance with agency and regulatory standards
Assist with patient care needs including ambulation, transfers, and positioning
Maintain clinical competency and perform other duties as assigned
Qualifications
Graduate of an approved School of Nursing Current Ohio RN license
Minimum 2 years of nursing experience in Medical/Surgical, Oncology, or community health nursing
Hospice experience preferred
Valid driver’s license required
Skills & Requirements Compassionate, patient-centered approach to care Excellent customer service, communication, documentation, and organizational skills
Detail-oriented and able to follow instructions
Ability to work independently with minimal supervision
Ability to travel to other sites as needed, including in inclement weather
Ability to lift up to 25 lbs.
Ability to sit, stand, walk, bend, twist, lift, and assist with patient transfers and positioning
