We are looking for a dedicated candidate to join our team as a Community Health Worker (CHW) puhservice patients in the [LOCATION] area. The Community Health Worker serves as a vital link between patients, healthcare services, and community resources, ensuring seamless access to care and improving the quality of service delivery. This role includes visiting patients in their homes and Assisted Living Facilities to facilitate visits with providers and specialty services and delivering medications and durable medical equipment (DME) to ensure timely and safe access to essential supplies.

Requirements:
Reliable transportation (personal vehicle) and a clean driving record.

• Work closely with care team members and the other staff to ensure coordinated patient care and compliance with the care plan.
• Communicate effectively via phone, email, Microsoft Teams, Athena, and in‑person.
• Facilitate and coordinate visits with PT’s, OT’s and providers in Assisted Living Facilities and patients’ homes.
• Deliver medications to patients’ homes, review instructions, and perform medication reconciliation.
• Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers.
• Set up training, instruct patients on the use of Remote Patient Monitoring (RPM) equipment, and provide ongoing support, education and troubleshooting.
• Deliver durable medical equipment (DME) to patients in homes and ALFs, ensuring proper assembly and functionality and providing training to patients and caregivers.
• Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling.
• Collect blood specimens during home or facility visits as needed to support patient care and diagnostics.

• Certified Medical Assistant, Par/router, or Licensed Nurse preferred. Certifications or licenses must be in good standing.
• Phlebotomy experience preferred; if not experienced, willingness to complete training and demonstrate competency.
• Previous experience in public health or home health preferred.
• Experience with DME delivery or logistics operations preferred.
• Proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records.
• Strong organizational and time‑management skills, with the ability to multitask and prioritize effectively.
• Strong written and verbal communication skills.
• Basic computer knowledge.
• Must be able to work independently with minimal supervision and direction.
• Good interpersonal and communication skills under all conditions.
• Flexible hours and travel between offices, facilities, etc.
• Licensed driver with an insured automobile in good working condition and a entfernen driving record.
