Responsibilities

  • Provides administrative support to Case Management teams as needed to support workflow process and outcomes, including answering multiple and designated telephone lines, prioritizing and routing calls, demonstrating effective interpersonal communication skills, and utilizing intermediate computer skills.
  • Collaborates with Case Manager / Social Worker to provide post‑acute referral for discharge needs.
  • Maintains and expands awareness of post‑acute care settings such as Skilled Nursing Facilities, Home Care Agencies, Infusion Services, Acute Rehab and Long Term Acute Care Facilities.
  • Coordinates patient transport when requested as well as medication assistance programs.
  • Assists with payor communications to care team and records these into the patient’s medical record.
  • Assists with front office duties as needed, entering referral notes, status of bed offers, and other details.
  • Independently and proactively initiates follow‑up with post‑acute referral settings to ensure a timely discharge.
  • Provides patient notices and patient choice timely and with explanation to patient and families based on mandated guidelines set forth by Medicare conditions of participation.
  • Performs all other duties as assigned.
Job Summary

Case Management Extenders role is to support various functions of Case Management that can be performed by a non‑licensed care team member. They assist the patient and follow the patient’s individualized plan of care with the guidance of a professional case manager.

Qualifications
  • Proficient with Microsoft Word and Excel.
