We are seeking a Part‑Time Nurse Practitioner (NP) to support Skilled Nursing and Post‑Acute facilities in Georgetown, SC.
Schedule & Environment- 3 days per week (Monday–Friday), 8:00 AM – 4:00 PM
- On-site at healthcare facilities
- No nights, weekends, or on‑call duties
- Evaluate and manage admissions, follow‑ups, and long‑term patients
- Provide chronic disease management and preventive care
- Collaborate with physicians and care teams
- Maintain accurate clinical documentation
- Serve as the clinical representative on‑site
- Active SC Nurse Practitioner license and board certification
- Strong clinical judgment and documentation skills
- Experience in post‑acute, hospital, SNF, or outpatient settings preferred
- Interest in geriatrics and continuity of care valued
- Competitive pay ($55–$65/hour)
