** Registered Nurse Case Manager
** to join our growing hospice team. The RN Case Manager coordinates and provides individualized end-of-life care to patients residing primarily in assisted living and memory-care communities.

The ideal candidate is clinically confident, highly organized, and committed to providing timely, responsive care. This position requires strong communication with patients, families, facility staff, physicians, and members of the interdisciplinary hospice team.##

Key Responsibilities
* Complete comprehensive nursing assessments and develop individualized plans of care.
* Provide skilled nursing visits based on each patient’s needs and ordered frequency.
* Monitor changes in condition and promptly communicate concerns to the hospice physician and interdisciplinary team.
* Manage pain, dyspnea, wounds, anxiety, agitation, secretions, and other end-of-life symptoms.
* Obtain and implement physician orders accurately and promptly.
* Coordinate medications, durable medical equipment, supplies, and other services.
* Educate patients, families, caregivers, and facility staff regarding disease progression, medications, symptom management, and end-of-life care.
* Provide emotional support and compassionate guidance to patients and families.
* Complete admissions, recertification assessments, routine visits, PRN visits, and death visits as assigned.
* Maintain accurate, timely, and complete electronic clinical documentation.
* Participate in interdisciplinary group meetings, care conferences, quality-improvement activities, and staff education.
* Collaborate with hospice aides, social workers, chaplains, physicians, facility staff, and community providers.
* Participate in the rotating after-hours and weekend on-call schedule.
* Ensure services comply with the plan of care, agency policies, and applicable Medicare, state, and accreditation requirements.
* Assist with additional clinical or operational responsibilities as needed to support patients and the hospice team.## Qualifications##

Required Qualifications
* Current, unrestricted Registered Nurse license in South Carolina.
* Minimum of one year of professional nursing experience.
* Current CPR certification.
* Valid driver’s license, reliable transportation, and automobile insurance.
* Ability to travel throughout the assigned service area.
* Strong clinical assessment, critical-thinking, and time-management skills.
* Ability to work independently while collaborating effectively with an interdisciplinary team.
* Professional communication and documentation skills.
* Ability to respond calmly and appropriately during changes in condition and end-of-life situations.
* Basic proficiency with electronic medical records and mobile technology.
