Center Well Home Health is seeking an experienced Home Health LPN/LVN to provide one-on-one nursing care in patients' homes under RN supervision. You will implement individualized care plans, educate families on disease management, and maintain detailed patient records to support recovery.
You will coordinate with physicians, clinical managers, and the care team while ensuring timely communication and patient safety across visits. Valid LPN/LVN license and CPR certification are required.
