Position: MDS Coordinator (RN or LVN) - Legend Greenville

MDS Coordinator

Texas RN or LVN License

Location:

2300 Jack Finney Boulevard, Greenville, TX 75402.

Responsibilities
  • Coordinate the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) / PPS and development of the Interdisciplinary Plan of Care.
  • Complete LTCMI (Long Term Care Medicare Information) or LOC (Level of Care) forms for the Medicare payment process when applicable.
  • Perform duties in a timely manner consistent with CMS guidelines and Legend Healthcare standards.
  • Establish and maintain positive relationships with all members of the health care team.
Qualifications
  • Current RN or LVN license in the state of Texas.
  • Experience with Electronic Health Records – preferably PCC experience.
  • Experience with the RAI process; MDS certification preferred.
  • Experience in a Skilled Nursing and Long Term Care facility preferred.
  • Experience as an MDS Coordinator preferred.
