SUMMARYProvides care management services for the Acquired Brain Injury (ABI) Waiver, which arranges for non-medical home care services as part of an overall case plan to maintain adults with physical and/or mental disabilities, age 18-64 inclusive, or 65+ if grandfathered, in the community. Additionally, provides these services for a family with a supervising relative who is temporarily incapacitated, unable to manage the household, or has a disability.
Performs all duties in a manner that fosters the achievement of the organization’s mission to identify choices and provide services to help people of all ages, abilities and incomes to live at home.
