- What You'll Do

* Provide hands-on, personalized care in patients' homes

* Manage and coordinate a caseload from start to finish

* Perform assessments and develop individualized care plans

* Monitor patient progress and adjust care as needed

* Collaborate with physicians and interdisciplinary teams

* Educate patients and families for long-term success

* Ensure compliance with all clinical and regulatory standards (including HIPAA)

- What You'll Need

* Active, unrestricted RN license

* 1+ year of clinical nursing experience

* Ability to travel within the coverage area (up to 50%)

* Reliable transportation, valid driver's license & insurance

* Strong clinical judgment and communication skills

* Ability to lift, move, and assist patients (50-100 lbs intermittently)

Preferred:

* Home Health or Hospice experience

* Experience with Home Care Home Base

* ADN or BSN
