Overview

The nurse navigator functions as a clinical resource and patient advocate across the continuum of cardiovascular care, supporting patients receiving care in the outpatient cardiac clinic and hospital-based procedural/interventional areas including Cardiology, Cardiac Catheterization Laboratory, Electrophysiology (EP), Interventional Radiology (IR), and related outpatient and inpatient services. The Nurse Navigator coordinates care for patients undergoing diagnostic and interventional cardiovascular procedures, promotes timely access to services, facilitates communication among multidisciplinary teams, and supports quality, safety, patient education, and continuity of care across the continuum of care.

The nurse navigator assists in the development of educational materials and program monitoring to promote coordination of care and patient satisfaction. The Nurse Navigator assists the clinical and leadership team with program development, expansion and successful achievement of programatic goals.

Responsibilities
  • Assist with clinical and administrative aspects, initiatives, and responsibilities associated with the YNHHS Heart and Vascular programs.
    • Assists physicians and providers with ongoing management of HVC patients.
  • Provides support to patient caregivers through guidance, direction and support in both inpatient, outpatient and community setting.
    • Coordinate and navigate care for cardiovascular patients across outpatient and inpatient settings.
  • Assist with HVC quality improvement process.
    • Participate in departmental specific quality improvement activity relevant to program and role.
  • Collaborates with physicians, nurses, advanced care providers and staff to assess educational needs of patients, families, and community.
    • Assess needs and collaborates with physicians and clinical staff to develop and provide ongoing heart and vascular education.
  • Medical record management.
    • Document all patient and family interactions in the electronic medical record according to established protocols and policy.
Qualifications Education

Bachelor of Science in Nursing (BSN) required. Masters degree in Nursing or Healthcare preferred.

Experience

Minimum of 3?5 years of cardiovascular nursing experience required. Prior experience in Cardiology, Cardiac Cath Lab, Electrophysiology, Interventional Radiology, Critical Care, or ambulatory cardiac services preferred. Experience coordinating complex patient care across outpatient and inpatient settings preferred. Knowledge of cardiovascular procedures, diagnostics, and treatment pathways required. Experience with electronic medical record systems (e.g., Epic) preferred.

Licensure

Connecticut RN license required. BLS and ACLS certification required.

Special Skills

Strong knowledge of cardiovascular disease processes, diagnostic testing, and interventional procedures. Excellent organizational, communication, and interdisciplinary collaboration skills. Ability to coordinate care across multiple departments and levels of care. Strong patient education and counseling abilities. Ability to prioritize and manage multiple patient populations and workflows simultaneously. Skilled in problem solving, critical thinking, and escalation of clinical concerns. Proficient in documentation and electronic medical record utilization.

Ability to analyze clinical and operational data to support quality improvement initiatives.

Physical Demand

Work is performed in both outpatient clinic and hospital procedural care environments. Frequent interaction with patients, families, physicians, and multidisciplinary healthcare teams. May require prolonged periods of sitting, standing, walking, and computer use. Exposure to hospital clinical environments, including procedural and inpatient care areas. May require flexibility in scheduling to support patient care coordination, activities, and procedural workflows.
