Location: Greenwich, CT

Patient: 2-year-old pediatric client

Clinical Needs: Tracheostomy care and temporary G-tube care

Home Environment: Beautiful family home, smoke‑free, with no pets

Family: Warm, supportive, and welcoming

• Active Connecticut LPN License
• New York applicants must maintain active LPN licenses in both New York and Connecticut
• Minimum of 1 year of pediatric experience in home care or acute care preferred
• Minimum of 1 year of experience providing pediatric trach and G‑tube care
• Current BLS Certification
• Authorized to work in the United States
• Physical Exam completed within the past year
• PPD/QuantiFERON completed within the past year, or Chest X‑Ray completed within the past five years
