What You'll Be Doing
As a Bilingual Patient Support Specialist (LPN), you'll provide hands-on clinical support while helping patients access the care and resources they need.
Responsibilities Include:
+ Deliver patient-centered nursing care in an ambulatory primary care setting
+ Conduct proactive patient outreach and engagement to improve continuity of care
+ Coordinate care plans and assist patients in navigating healthcare services
+ Respond to patient phone calls and electronic messages while escalating concerns when needed
+ Perform clinical testing and nursing procedures within LPN scope of practice
+ Support primary care and specialty care programs
+ Maintain exam rooms, clinical supplies, and medical equipment
+ Accurately document patient care in electronic health records (EHR)
+ Participate in population health and quality improvement initiatives, including UDS and HEDIS measures
+ Ensure compliance with HIPAA, OSHA, CLIA, HRSA, and Joint Commission standards
+ Collaborate with providers and interdisciplinary care teams to deliver exceptional patient outcomes
Qualifications
Required:
+ Graduate of an accredited Licensed Practical Nurse (LPN) Program
+ Active Connecticut LPN License
+ Current CPR/BLS Certification
+ 2-4 years of clinical experience in:
+ Primary Care
+ Ambulatory Care
+ Acute Care
+ Strong care coordination and patient communication skills
+
Experience with Electronic Health Records (Epic experience preferred)
+ Ability to thrive in a fast-paced healthcare environment
+ Bilingual in Spanish and English

This is a Contract to Hire position based out of Greenwich, CT.
Pay and Benefits
The pay range for this position is $31.25 - $36.00/hr.
Eligibility requirements apply to some benefits and may depend on your job classification and length of employment. Benefits are subject to change and may be subject to specific elections, plan, or program terms. If eligible, the benefits available for this temporary role may include the following:
