Yale New Haven Health in Connecticut is seeking a registered nurse to contact patients discharged from the Emergency Department and verify understanding of discharge instructions.
You will identify ED patients from EPIC reports, perform follow-up calls within 48 hours, provide clarification as needed, and support service recovery with compassionate communication. Requires CT RN license or permit, BLS, and at least 2 years in the ED, plus proficient computer and telephone skills.
