ResponsibilitiesProvide primary care across multiple family medicine sites in several South Carolina counties. Develop and oversee a community-based primary care program focused on family-centered, holistic care. Design an integrated delivery model emphasizing prevention, health maintenance, education, and chronic disease management. Collaborate with a multidisciplinary team to improve community health outcomes.
QualificationsThe ideal candidate is committed to patient-centered care, with strong communication skills, clinical expertise, and knowledge of current best practices in primary care and chronic disease management.
RequirementsGraduate degree from an accredited Nurse Practitioner program. Active South Carolina licensure. At least two years of experience, and relevant certification.
SkillsEffective communication, confidentiality, critical thinking, and problem-solving skills.
Physical RequirementsTypical clinical activities, moderate computer use, and minimal travel.
