We are seeking a qualified Nurse Practitioner that specializes in wound care to serve our patients throughout the Low Country area. As a provider, you play a critical role in delivering exceptional patient care across various settings, including homes, facilities, and telehealth. You collaborate with multiple healthcare professionals to ensure comprehensive and effective patient management.

Our providers conduct virtual patient visits from the comfort of their own homes as well as in-person visits in various settings such as homes and facilities. This is a full-time salary‑based position, with the opportunity to receive a monthly bonus.

Shift(s) Available
  • 8‑hour shift (Monday‑Friday; 8:00am‑5:00pm)
  • 10‑hour shift (Monday‑Thursday; 7:00am‑6:00pm)
  • 10‑hour shift (Tuesday‑Friday; 7:00am‑6:00pm)

What We Are Looking For ?

We are seeking a Nurse Practitioner to join our growing primary‑care family. A successful provider will be able to perform essential duties and responsibilities below. Areas Of Responsibility
  • Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
  • Coordinate care activities with other members of the interdisciplinary team to ensure continuity of care and optimal outcomes.
  • Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, and other team members to coordinate patient care.
  • Participate in interdisciplinary team meetings to discuss patient progress, treatment plans, and care coordination efforts.
  • Foster effective communication and collaboration to facilitate seamless patient management and teamwork.
  • Meet staff efficiency expectations while providing high‑quality care to patients in various settings, including homes, facilities, telehealth, clinics, and specialties.
  • Document care management activities accurately and efficiently to support patient care plans and regulatory compliance.
  • Provide patients with education and resources related to their conditions, treatment plans, self‑management strategies, and preventive care measures.
  • Offer support and guidance to patients and their families to enhance understanding of healthcare options and promote active participation.
  • Adhere to all relevant healthcare regulations, policies, and procedures, including HIPAA and Medicare guidelines.
  • Monitor quality measures and gaps in care, identifying opportunities for improvement and implementing corrective actions as needed.
Qualifications
  • NP, PA, or physician with a valid license to practice in the state of employment; license must be in good standing.
  • Demonstrated experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care settings.
  • Strong communication, collaboration, and interpersonal skills for effective interdisciplinary teamwork.
  • Ability to evaluate high‑risk patients, develop care plans, and coordinate activities to meet patient needs and improve outcomes.
  • Commitment to delivering compassionate, patient‑centered care and promoting a culture of excellence.
  • Strong organizational and time‑management skills; ability to multitask and prioritize effectively.
  • Ability to read and communicate clearly, both in writing and verbally.
  • Basic computer proficiency.
  • Demonstrated leadership skills and ability to foster a cooperative work environment.
  • Team player able to manage multiple responsibilities and demonstrate sound judgment.
  • Must be able to work flexible hours and travel between offices, facilities, etc.; licensed driver with an insured automobile in good working order.
