Registered Nurse (RN) Case Manager – Hospice

Marlborough, MA (Serving Worcester, Middlesex, Norfolk & Bristol Counties)
Full-Time | Monday–Friday, 8:30 AM–5:00 PM

Position Overview

As an RN Case Manager
, you will play a vital role in supporting patients and their families through the end-of-life journey. You'll provide skilled, compassionate care in a variety of settings, including private homes, nursing homes, and assisted living facilities.

This is a field-based position requiring travel throughout our service area.

Key Responsibilities
  • Coordinate and deliver individualized care for hospice patients and their families
  • Provide direct nursing care in alignment with the Interdisciplinary Plan of Care
  • Perform ongoing patient assessments and adjust care plans as needed
  • Collaborate with interdisciplinary team members to ensure optimal patient outcomes
  • Coordinate and oversee care provided by vendors and community partners
  • Participate in patient admissions and provide nursing services as needed outside of standard hours
  • Take part in an on-call rotation
    , including emergency backup and occasional holiday coverage
Qualifications
  • Active Registered Nurse (RN) license in the Commonwealth of Massachusetts (in good standing)
  • Prior hospice experience preferred
  • Ability and willingness to travel within the service area using a reliable, insured, and registered vehicle
  • Experience with Point of Care EMR documentation
  • Current CPR / Basic Life Support (BLS) certification
  • Strong clinical, organizational, and communication skills
  • Ability to work independently while collaborating effectively with a care team
