As an RN Case Manager
, you will play a vital role in supporting patients and their families through the end-of-life journey. You'll provide skilled, compassionate care in a variety of settings, including private homes, nursing homes, and assisted living facilities.
This is a field-based position requiring travel throughout our service area.
Key Responsibilities- Coordinate and deliver individualized care for hospice patients and their families
- Provide direct nursing care in alignment with the Interdisciplinary Plan of Care
- Perform ongoing patient assessments and adjust care plans as needed
- Collaborate with interdisciplinary team members to ensure optimal patient outcomes
- Coordinate and oversee care provided by vendors and community partners
- Participate in patient admissions and provide nursing services as needed outside of standard hours
- Take part in an on-call rotation
, including emergency backup and occasional holiday coverage
- Active Registered Nurse (RN) license in the Commonwealth of Massachusetts (in good standing)
- Prior hospice experience preferred
- Ability and willingness to travel within the service area using a reliable, insured, and registered vehicle
- Experience with Point of Care EMR documentation
- Current CPR / Basic Life Support (BLS) certification
- Strong clinical, organizational, and communication skills
- Ability to work independently while collaborating effectively with a care team
