Position Overview

Seeking a skilled Registered Nurse (RN) to support home health admissions, case management, and LPN oversight. This role offers autonomy in scheduling patient visits while ensuring high-quality, patient-centered care in alignment with clinical and regulatory standards.

Key Responsibilities
  • Conduct admissions for home health services, including comprehensive patient assessments and OASIS documentation.
  • Provide case management oversight, ensuring continuity and quality of patient care.
  • Supervise and support LPN staff, ensuring adherence to care plans and clinical standards.
  • Develop and manage individualized care plans based on patient needs and physician orders.
  • Perform clinical assessments and ongoing evaluations to monitor patient progress.
  • Ensure compliance with regulatory requirements and documentation standards.
  • Collaborate with interdisciplinary healthcare teams to coordinate patient care effectively.
  • Manage time efficiently to meet productivity expectations and deliver timely care.
Required Qualifications
  • Active Registered Nurse (RN) license
  • Current CPR certification
  • Proven experience with OASIS documentation
  • Strong background in home health admissions and case management
Preferred Qualifications
  • Experience with Homecare Homebase (HCHB) system
  • Strong expertise in wound assessment and wound care management
  • Excellent communication and time management skills
  • Ability to work independently with strong clinical judgment
Compensation & Additional Information
  • Mileage Reimbursement:
  • Provided between patient visits (excluding initial commute).
  • First visit mileage adjusted based on distance between home and agency.
Work Environment & Dress Code
  • Flexible, field-based role with independent scheduling
  • Dress Code:
    Grey or blue scrubs preferred (not mandatory); business casual acceptable
