** Position Summary
** As a
** Wound Care Nurse (LPN)**, you will be responsible for providing specialized skin and wound care to residents under the direction of the physician, Director of Nursing, and/or Medical Director. You will play a key role in promoting healing, preventing complications, and improving overall resident outcomes.### ###
** Key Responsibilities
*** Provide
** direct wound care and skin treatments
** in accordance with physician orders
* Assess, monitor, and document wound progress and treatment effectiveness
* Assist in developing and
** modifying individualized care plans
*** Collaborate with the interdisciplinary team to ensure optimal outcomes
* Educate staff on proper wound care techniques and prevention strategies
* Maintain accurate and compliant clinical documentation
* Ensure adherence to all regulatory, safety, and infection control standards### ###
** Qualifications
* ** Active, unencumbered
** LPN license in South Carolina
*** Graduate of an accredited
** Practical Nursing program**
* ** Current CPR certification required
*** Ability to work
** 12-hour shifts
*** Experience with
** Point Click Care  (PCC) preferred
*** Prior wound care experience strongly preferred
* Strong clinical judgment and attention to detail
* Passion for delivering compassionate, resident-centered care### ###
