Program Coordinator

Purpose

To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.

• Must successfully complete required pre‑employment background and drug screening.
• Must possess a valid driver’s license with all necessary endorsements and a minimum of 3 years of driving experience.
• Must possess advanced computer skills.
• Must possess a high school diploma or equivalent (Bachelor’s degree preferred).
• Experience working in the field of intellectual/developmental disabilities preferred.
• Minimum one year of experience in writing program plans, case notes or other relevant documentation required.

• Responsible for the development, implementation and coordination of individual program plans.
• Develop, implement and monitor individualized training programs to meet identified priority training needs.
• Ensures all plans are person‑centered and reflect the individual’s hopes and dreams through active participation wherever possible.
• Ensure all documentation is accurate and complete according to regulatory requirements.
• Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed.
• Perform other duties as assigned.
• Mandated to report any instances of abuse, neglect or exploitation that are witnessed or experienced.
