NUMBER OF VACANCIES: 1
JOB SUMMARY: Establishes Family Partnership Agreements with families. Assists families with goal setting. Develops service plans for each family as needed. Coordinates services to provide assistance, information, referrals, and follow-ups with families. Assists in facilitating parent engagement activities. Coordinates training/workshops for families. Conducts children’s application intake and processes applications. Enters information into the data management system, maintains accurate up-to-date files, and provides data reports as required.
Makes periodic home visits as needed.
QUALIFICATIONS: Family Development Credential (FDC), Associate degree in Social Work, Family Development, Human Services, or related fields preferred. Two years of experience in family services or equivalent experience in social services and/or developing and implementing a parent-related program is preferred. Must be sensitive to the needs of low-income families and have previous case management experience. Knowledge of available community resources is desired.
Must have good written and oral communication skills as well as the ability to plan and work independently. A valid South Carolina driver’s license with an acceptable driving record is required.
