Registered Nurse / RN Team Director, Hospice

Compensation: $80,

Schedule:

Monday to Friday, on call as needed

• Associate degree preferred
• Certified Hospice Nurse (CHPN) preferred
• Three+ years of experience in hospice, home health, general medical-surgery, oncology, or patient population‑specific nursing
• Minimum of one year of supervisory experience
• Valid CPR certification while employed in a clinical role (preferred)
• Licensed driver capable of traveling to all business locations
• Meet all state regulations and requirements for the program

• Supervise patient and family care team members in collaboration with program site leadership
• Handle scheduling, payroll and related administrative duties
• Ensure all initial assessments and required reassessments are completed by qualified team members within required time frames, including documentation
• Oversee and support Interdisciplinary Group (IDG) for development, implementation, and review of patient care plans
• Collaborate with site leadership to monitor, manage, and mentor staff on patient care activities, productivity, and documentation per policies
• Participate in Back to Bedside efforts, reinforcing case management skills and protocol adherence across disciplines
• Support staff education initiatives with site leadership
• Facilitate IDG meetings and provide ongoing support such as triaging calls, caller assistance, and performing quality calls
• Participate or lead quality improvement activities, including data collection and aggregation
• Reinforce internal and external communications
