Job Overview:
We are currently seeking a dedicated and experienced Physical Therapist (PT) to join our team for a 13-14-week contract in Gresham, OR. This position focuses on helping aging adults regain mobility and independence.
Key Responsibilities:
- Evaluate patients’ physical conditions and develop individualized treatment plans
- Provide direct physical therapy services to patients in accordance with the plan of care
- Collaborate with a multidisciplinary team including OTs, PTAs, and nursing staff
- Monitor patient progress and adjust therapy plans as needed
- Document all patient interactions, progress notes, and outcomes in a timely manner
- Educate patients and families on at-home exercises and post-discharge care
- Maintain compliance with facility policies, federal and state regulations, and professional standards

QualificationsRequirements:
- Active and valid Oregon Physical Therapist (PT) license
- Graduate of an accredited Physical Therapy program
- Strong clinical reasoning and decision-making skills
- Ability to work independently and as part of a care team
- Excellent interpersonal and communication skills
- Familiarity with electronic documentation systems (EMR)
