Description

The Átawišamataš Home Visitor (AHV) will provide direct outreach to the CTUIR community, client families, providers, departments, and organizations involved in direct services, as well as community leaders and advocates. The AHV will conduct home visits using Johns Hopkins University’s Family Spirit evidence‑based curriculum. The Tribal Maternal, Infant, and Early Childhood Home Visiting (TMIECHV) grant funds this program through October 1, 2028.

The role promotes healthy pregnancies, breastfeeding, and parent‑infant attachment, offering prenatal, labor, and postpartum support to mothers and caregivers in alignment with Yellowhawk’s Mission and Vision statement.

Essential Job Functions
  • Work under the direction of the Átawišamataš Program Manager to provide home visiting services to families and various settings.
  • Collaborate with Program Manager and other care teams to coordinate care, advocate patient needs, and develop comprehensive care plans.
  • Present, communicate, and teach to small and large groups in a clear, engaging manner.
  • Perform home visits to assess patient and family healthcare and social needs, promoting positive parenting and healthy living.
  • Understand grant objectives, strategies, and manage collection and maintenance of grant data and reporting requirements.
  • Complete Community Health Worker Certification and maintain adherence to policies, procedures, and program strategies.
  • Maintain certifications in Family Spirit Curriculum and Fatherhood is Sacred/Motherhood is Sacred, following Yellowhawk Home Visiting Policy and Procedures.
  • Collaborate with other departments to maintain communication and good working relationships within the organization, ensuring quality health care service for patients.
  • Co‑operate with Yellowhawk Public Relations Department to create social media campaigns, advertising, and print materials, adhering to Communications Policies.
  • Provide service link and referral resources to assist families with their healthcare and social needs.
  • Represent Yellowhawk with a respected and viable presence at community events, within the clinic, and at conferences and events outside of the CTUIR boundaries.
  • Maintain confidentiality in accordance with the Yellowhawk Tribal Health Center Policy Manual, HIPAA and the Privacy Act.
  • Perform other duties as assigned.
Requirements
  • Associate's Degree in Social Work, Early Childhood Education, Psychology or related field; OR High School Diploma and 2 years relevant work experience with youth or families in Tribal communities.
  • General understanding of trauma‑informed approaches to working with women, children and families.
  • Working knowledge of Microsoft Office programs and/or other relevant software to meet the needs of various standards in both clinical and grant‑related documentation.
  • Must possess high‑level verbal and written communication skills.
  • Demonstrated ability to work effectively in a team setting with diverse membership.
  • Must complete Community Health Worker certification within 18 months of hire (dependent upon training availability).
  • Must possess or receive CPR and First Aid certification within ninety (90) days of employment.
  • Must possess a valid driver's license and maintain insurance requirements to operate General Services Administration vehicles.
  • Must submit to and pass a reference and criminal background check.
  • Must submit to and pass a pre‑employment drug and alcohol screening.
Preferred Qualifications
  • Bachelor's Degree in Social Work, Early Childhood Education, Public Health, or related field.
