Native American Rehabilitation Association Northwest (NARA NW) is seeking a Patient Navigator to support patients in overcoming barriers to care and to coordinate services across health and community programs. You will work onsite in Gresham/Portland, guiding patients, arranging transportation, and linking them to SNAP, WIC, and OHP while promoting self‑sufficiency.
The role requires a High School Diploma or GED, CHW and CPR certifications, a valid driver’s license, and willingness to travel
