Position Summary

The Case Manager provides individualized, trauma-informed support to families residing in Just Compassion’s 50-unit family shelter in Gresham, Oregon. This role supports families in stabilizing, accessing services, and transitioning into safe and permanent housing.

Working collaboratively with participants, the Case Manager develops and implements personalized service plans focused on housing stability, employment, education, and overall family well-being. The position requires strong advocacy, coordination with community partners, and navigation of complex systems including Coordinated Entry, TANF, SNAP, OHP, childcare assistance, and other public benefit programs.

Essential Duties
  • Provides individualized case management and housing navigation services to support families in achieving housing stability.
  • Coordinates access to community resources and public benefit systems to promote long-term family well-being.
  • Maintains accurate documentation and ensures compliance with program standards, confidentiality requirements, and funding guidelines.
Job Functions
  • Conducts intake assessments and collaborates with families to develop individualized service plans focused on housing stability and self-sufficiency.
  • Provides ongoing case management, advocacy, and goal tracking for families experiencing homelessness.
  • Assists families in navigating housing opportunities, including Coordinated Entry referrals, landlord engagement, rental applications, and move-in coordination.
  • Supports participants in obtaining necessary documentation such as identification, income verification, and vital records.
  • Assists families in accessing public benefits including TANF, SNAP, OHP, childcare assistance, and other stabilization resources.
  • Maintains current knowledge of local housing systems, rental assistance programs, and community-based supports.
  • Collaborates with internal team members, including youth services, childcare, and behavioral health staff, to ensure holistic family support.
  • Advocates for families within schools, housing systems, public agencies, and other institutions.
  • Empowers families to build self‑advocacy skills and navigate systems independently over time.
  • Provides trauma‑informed, strengths‑based support and crisis intervention as needed.
  • Participates in multidisciplinary case review meetings and team coordination efforts.
  • Maintains consistent communication with shelter operations staff regarding participant progress and needs.
  • Maintains accurate and timely case notes, service plans, and outcome data in HMIS or internal databases.
  • Tracks participant progress toward housing stability, employment, and wellness goals.
  • Ensures compliance with confidentiality requirements, HIPAA standards, and applicable funding regulations.
  • Contributes to program reporting and outcome tracking for internal evaluation and funder requirements.
  • Performs additional duties as assigned in support of program operations.
Qualifications
  • Bachelor’s degree in Social Work, Human Services, or related field; or equivalent professional experience.
  • Minimum of two (2) years of experience providing case management, housing navigation, or family advocacy services.
  • Knowledge of trauma‑informed care, harm reduction, and strengths‑based approaches.
  • Familiarity with Coordinated Entry and Oregon benefit programs (TANF, OHP, SNAP).
  • Strong interpersonal, organizational, and problem‑solving skills.
  • Ability to manage multiple priorities in a fast‑paced environment while maintaining professionalism and empathy.
  • Valid driver’s license and reliable transportation required.
